To obtain an electronic, printable Certificate for Cardinal Health medical products, enter the Cardinal Health product number (as ordered) and the product lot number (as it appears on the product labeling) into the fields below. Select the type of certificate desired in the drop down and click the 'print and preview' button to generate the certificate.

NOTE: All fields are required to generate the certificate, and only one product is able to be generated at a time for one or up to five lots. Click the "additional lots" tag to open more fields to populate additional lot numbers for the same product catalog number. Only one lot per entry space.
